Updated Advice & What You Need To Know
ABM’s latest guidance positions cold therapy as first-line support for breast inflammation—helping to reduce swelling, tenderness, and pain from engorgement, early inflammatory mastitis, and ductal narrowing (often called blocked ducts).
Cold Therapy
Use cold therapy after breastfeeding or pumping to reduce swelling and soothe tenderness—especially through periods of oversupply, weaning, or return-to-work pumping. It’s a simple, evidence-based way to feel better while protecting your supply.
Warm Therapy
A little warmth just before a feed or pump can soften the breast and encourage let-down for a smoother latch or more comfortable pumping session. Keep heat short and purposeful; during active breast inflammation, prolonged warmth is not recommended.

FAQ
EXPLORE FREQUENTLY ASKED QUESTIONS
Why does hot and cold therapy matter for breastfeeding and breast care?
Dual thermal therapy offers effective support for common breast changes in pregnancy and postpartum. For acute inflammation; engorgement, ductal narrowing (blocked ducts), inflammatory mastitis, cold therapy helps to reduce swelling and tenderness. Warm therapy may assist let-down immediately before feeding or pumping. Current clinical guidance prioritises cold therapy during inflammatory episodes and cautions against sustained heat on inflamed breast tissue.
How do I use Mammae's hot & cold thermal therapy breast packs?
Use warm therapy before breastfeeding or pumping to support milk flow and comfort. Use cold therapy after feeds or between feeds to soothe breast pain, reduce inflammation and engorgement.
How do I heat my breast pack safely?
Follow the heating time instructions according to the WATTS of your microwave. This information is outlined inside your Care Card. Place the pack flat in a clean microwave; heat in short 5 minute bursts and test on your inner wrist before use. Do not overheat or use an oven.
How do I cool my thermal therapy breast pack effectively?
Chill in the freezer for a minimum 30 minutes on a clean surface then insert into the bamboo cover prior to application. Apply in short sessions, with regular skin checks. Cold therapy is preferred for acute inflammatory breast pain and engorgement.
Can I use the hot and cold thermal therapy breast packs with a breast pump?
Yes, the large 6cm opening is thoughtfully designed to fit around all universal electric or manual breast pump flanges. They can make pumping more comfortable and efficient. Try a brief pre-pump warm-up (gel pack in the bamboo cover, placed around the breast or over the outside of the flange. This can help let-down and softening. After expressing, switch to a cold therapy pack for 10–15 minutes to calm post-pump swelling and tenderness.
Do I have to use the bamboo cover?
Yes, it is best practice to use the included GOTS & OEKO-TEX certified bamboo terry cover, however it also comes down to personal preference and temperature guidelines. The ultra-soft and certified bamboo material protects delicate postpartum skin from direct heat or cold, improves comfort, and keeps the packs hygienic. (Our covers are known to be buttery-soft and easy to wash.)
How do I clean the gel packs?
Spot-clean only. Wipe the outside surface with a soft cloth and mild soap, then air-dry fully. Do not machine-wash or tumble-dry.
How do I wash the bamboo covers?
Delicate machine-wash. Cold, gentle cycle, with mild fragrance-free detergent. Avoid bleach or fabric softeners. Line-dry flat or tumble-dry low.
How long should I apply the hot or cold?
Short sessions work best—around 10–15 minutes with regular skin checks. Reapply cooling as needed. Always use the cover to mitigate sensitivity or burns and stop if skin feels too hot, too cold, numb, or uncomfortable. Avoid prolonged heat therapy on inflamed tissue. (ABA Guidelines.)
Can thermal therapy help with mastitis or “blocked ducts” (ductal narrowing)?
Yes, with current updates from ABM (2022). For inflamed tissue, prioritise cooling. Avoid deep massage and avoid “emptying” the breast; instead, feed/pump to comfort on your normal schedule. Brief warmth pre-feed is fine if it helps let-down. Seek medical review if fever or systemic symptoms persist >24 hours. (ABM Protocol; bfmed.org.)
Is hot and cold thermal therapy safe in pregnancy and postpartum?
Yes, when used as directed. Avoid extreme temperatures, direct skin contact, and use on broken skin. If you have reduced skin sensation, dermatologic conditions, or a history of vasospasm, take extra care and follow your clinician’s advice. (NICE & ABM aligned.)
When shouldn’t I use hot & cold thermal therapy packs?
Do not use on numb areas, open wounds, or active skin infections, or if the pack feels overheated/too cold to tolerate.
When should I use warm therapy?
Use brief, gentle warmth right before a breastfeed or pumping session to support let-down and ease initial tenderness. Keep applications short (5–10 minutes), always with the bamboo cover and regular skin checks. For nipple vasospasm/Raynaud’s, gentle warmth after feeds may soothe. Outside the pre-feed window, avoid prolonged heat on inflamed tissue; for engorgement or inflammatory breast pain, prioritise cooling between feeds.
When should I use cold therapy?
Use our Cold Thermal Therapy breast packs after feeds or between feeds any time your breasts feel inflamed e.g. with engorgement, ductal narrowing (formerly “blocked duct”), or inflammatory mastitis.
Cooling helps lower swelling, heat and tenderness. Keep milk removal physiological and comfortable (feed/pump to comfort—don’t try to “empty”) avoid deep massage.
Apply in short, frequent sessions with the cover on (e.g. 10–15 minutes; reapply as needed).
Reserve warmth only briefly before a feed/pump if it helps let-down, otherwise stick with cooling for inflammation.
This approach reflects current ABA advice and ABM Protocol #36 (ice/NSAIDs to reduce oedema; heat may worsen symptoms).
